I/we understand that to evaluate my credit application and to continue monitoring my credit status, as well as my on going eligibility for credit, and for the purposes I/we consent to below, a personal information file containing credit and other personal information will be created. I/we understand that if I/we withdraw consent at anytime to the evaluation of my credit application, to the monitoring of my credit status or my on going eligibility for credit, N. Tepperman Limited may no longer be able to maintain my credit account. I/we consent to, and accept this as written notice of, your obtaining, disclosing or exchanging credit, personal or other information about me (including information contained in my personal information file) at anytime, from, to or with any credit bureau, personal information agent, credit grantor or insurer, my employer or other person in connection with any relationships between me/us or those established by your mutual agreement.

By submitting this agreement I/we agree that, as a submitting applicant, I will jointly and individually be liable for any use of the Tepperman's charge account and all amounts charged to the Account.
